East Liverpool City Income Tax Office

The City Income Tax Office is responsible for collection of city taxes including income tax (1.5%), Hotel/Motel tax (3%), and Bed tax (3%). Mandatory filing was enacted in 1989. Income tax is based on gross income of wages, net profit of businesses, and net profit of rental income. Income tax is imposed on all residents of the city, resident businesses and non-resident businesses located in the city, and non-residents working in the city.

Declaration of Exemption Form

Declaration of Estimated Tax

- The purpose of this form is to file a declaration of your estimated tax that will be due for the current year. It is used when your employes does NOT withhold city tax from your pay. This form is also used to make miscellaneous payments to your account when a declaration has NOT been made first.

How to prepare this form (Line #):

Enter the amount of gross income you expect to receive from all emplyees not withholding city tax.
Multiply Line 1 by 1.5% (tax rate) and enter here. This is the amount of tax that will be owed based upon your estimated income for the year.
Enter any credits from prior year.
Add $25.00 late filing penalty if this is not submitted by April 30th.
Take amount on Line 2 and divide by 4. This equals quarterly payment amount. Subtract from this any amount on Line 3 and add any amount on Line 4. This is the total due for your 1st quarter payment. This amount must be paid with this declaration and sumbitted by April 30th.
